Membership suits patients on a long medical weight-management arc, patients maintaining results after bariatric surgery, and patients managing insulin resistance, type 2 diabetes or related metabolic conditions where periodic specialist review is clinically indicated.
Membership is not appropriate for every enquiry. A Meridian physician will determine whether the programme is the right fit at first consultation, or whether a different pathway, surgical or medical, is more appropriate.
A referral is helpful but not required to enquire. Medicare rebates apply where the patient holds a valid GP referral and the service is eligible.
Three steps to enrolment.
Initial consultation
You meet a Meridian physician. The clinical question is examined in full, history, investigations and any current medication.
Pathway recommendation
If membership is the right pathway, the physician explains the programme, the medication plan, the review schedule and what is expected of you.
Twelve-month arc
Care unfolds across the year. Quarterly physician review. Dietetic and psychology input as required. The patient app holds the file in one place.
Investigations held in the file, not chased between providers.
Periodic blood work, body composition, lipid and HbA1c monitoring, and other investigations clinically indicated for the medication and the patient. Pathology is requested through the practice and returns into the patient's clinical file.
Where additional imaging or specialist investigation is required, the practice arranges it within Brenan Place or with established referral partners.
If the medical pathway reaches its limit, the surgical pathway is in the next room.
Some patients on the metabolic pathway will, over time, become candidates for bariatric surgery. The decision is reached through the same multidisciplinary review that opens the membership, and the surgical team is already known to the patient.
The transition is held within one practice. There is no second referral, no new file, no repetition of history.
For post-bariatric patients, membership holds the years that follow.
Bariatric surgery is a clinical event. The decade after is the patient's. Meridian Membership is open to post-bariatric patients of the practice for ongoing physician review, dietetic support, supplement monitoring, and management of weight regain or metabolic drift.
Care is coordinated with the operating surgeon and shared with the patient's GP.
The same clinicians, the same file, every visit.
Membership is held by named clinicians. The physician who opens the programme is the physician who reviews you across the year. The dietitian and psychologist remain consistent. The patient app holds the longitudinal record.
Membership can be paused or closed at any time. Closure does not affect the patient's standing with the practice or any active surgical care.
Meridian Membership is operated by Meridian Health under the supervision of consultant physicians.
It is not a wellness subscription, a coaching product, or a direct-to-consumer medication service.
