How the Meridian patient app collects, uses and protects your information.

Your record stays inside the practice.
The Meridian patient app is operated by Meridian Health. The information you enter sits within the practice’s clinical system and is used only to support your care. It is not sold, shared with advertisers or used for any purpose outside your care.
What we collect and why.
Information you provide
To use the app you sign in with a patient number and a one-time setup code issued by the clinic. Within the app you may record information about your care, such as your weight, fluid intake, exercise, calorie and nutrition entries, supplements, appointments and reminders, along with any notes or questions you choose to save. The app also keeps track of your progress through your care journey.
How your information is used
Your information is used only to support your care with the practice. It helps your care team understand your progress, and it lets the app release the right reading, trackers and prompts at the right stage of your journey. It is not used for marketing or advertising.
How it is stored and protected
Your information sits within the practice’s clinical system, hosted on secure cloud infrastructure with servers located in the United States. Our hosting provider stores data on the practice’s behalf and is not permitted to use it for its own purposes. Connections between the app and the practice are encrypted while in transit. Access on your device is tied to your patient number and your one-time setup code. After the first sign in, your device is securely linked so you do not need to enter the code again.
How long it is kept
Health information is kept for as long as the law requires: at least 7 years from your last visit or entry, or if you were under 18 at the time, until you turn 25 (whichever is later), in line with the Health Records Act 2001 (Vic). Everyday tracker entries that are not part of your health record can be deleted sooner on request. When a record is no longer required it is securely destroyed, and a record of that destruction is kept.
What we do not do
We do not sell your information. We do not share it with advertisers. We do not show third-party advertising in the app, and we do not use your information for any purpose outside your care.
Overseas disclosure and AI features
The optional food-photo and nutrition-estimate features in the calorie tracker use artificial intelligence. When you use them, the photo or description you provide is sent to OpenAI, a service provider located in the United States, so it can estimate nutrition information. This is the only situation in which information you enter in the app is disclosed overseas. Your name and patient number are never sent with the request. The app asks for your consent before the first use; the features do not run without it, and you can withdraw consent at any time from the app’s Privacy screen. OpenAI processes the information only to deliver the feature and does not use it for its own purposes.
Deleting your information
You can pause or delete the app at any time, with no effect on your clinical care. Removing the app from your device removes the local copy of your information. To request deletion of the information held for you in the app, contact the practice using the details below and we will action your request. See your privacy choices for the options available to you.
Changes to this policy
If this policy changes, we will update this page and revise the date shown above. Where a change is significant, we will make it clear to patients.
Questions about your privacy.
If you have questions about how your information is handled, or you believe we have mishandled your information, contact the practice first during business hours. We will acknowledge a complaint within 7 days and aim to resolve it within 30 days.
If you are not satisfied with our response, you can escalate your complaint to the Health Complaints Commissioner (Victoria) at hcc.vic.gov.au or 1300 582 113, the Office of the Australian Information Commissioner at oaic.gov.au or 1300 363 992, or the Office of the Victorian Information Commissioner at ovic.vic.gov.au for matters under Victorian information law.
